Add licence template to all export formats

Description

As shown by Elke, we need to display licence information and attribution description at the beginning of the exported versions of the deck.

This is happening for PDF, ePub, SCORM and xAPI launch package. I think that's everything.

  • First step could be: putting just the URL of the exported deck into the footer of the PDF or ePUB.

  • Second step: prepending a page with full attribution and CC button linking to the license URL.
